Supported Independent Living
The Florida Institute for Neurologic Rehabilitation, Inc. provides a
Supported Independent Living Program which offers an opportunity for individuals
with brain injuries and other neurologic impairments to be afforded supported
living in a dignified community setting at a reasonable daily cost. The program
is designed to provide normalized community living and meaningful daily
activities along with the support and supervision necessary to ensure safety and
security.
The Supported Independent Living Program offers the client:
- A program designed and managed by nationally recognized experts in the
field of brain injury rehabilitation.
- Professionally trained staff that provide daily support and supervision.
- Daily community integration activities.
- Supervision 24 hours per day.
- Staff to client ratios based on individual needs.
- Supported residential living based on supervision and intensity of needs.
- Opportunities to access resources available in the Central Florida area
for recreational and therapeutic programming (i.e., voc/tech community
college, parks, shopping, etc.)
The Supported Independent Living Program offers the funder:
- A comprehensive per diem that is individually negotiated based on the
client's needs. The per diem includes daily support and supervision as well
as therapeutic activities, community living training, daily vocational/avocational
activities, transportation and room & board.
- A daily rate that is typically lower than competitive programs.
- A proactive attempt to increase the client's independence thereby allowing
us to reduce the daily rate.
- A variety of medical, rehabilitation, educational, and vocational options
are available on-site and in the Central Florida area.
- Internal Case Management to insure quality of service as well as cost
effectiveness.
- Recognized experts in brain injury rehabilitation managing day-to-day
operation.
- An individual extended service plan with updates being provided in writing
on a regular basis.
- Program activities and outcomes that are data based and reviewed on a
monthly basis.
- Active involvement of the client, family and funder in decision making.
- Utilization review conducted weekly to evaluate levels of service and
supervision and the cost of care.
The Florida Institute for Neurologic Rehabilitation's innovative and cost
conscious Supported Independent Living Program is designed to provide
individualized programs that teach the skills needed to become more independent
and provide necessary support and supervision. The overall goals of this program
is increased self sufficiency and improved quality of life in a safe and secure
environment.
