All referrals for rehabilitation are reviewed on an individual basis.  The criteria listed below are general guidelines and do not individually qualify or disqualify eligibility.

  • Neurologic dysfunction
    • Such as:
      • Traumatic Brain Injury
      • Neurologic Injury/Disorder
      • Central Nervous System Injury
    • As the result of:
      • Closed or penetrating head injury
      • Central nervous system disease
      • Brain tumors
      • Cerebrovascular accidents
      • Exposure to neurotoxin
      • Metabolic disorders
      • Anoxic/hypoxic  encephalopathies
      • Varied encephalopathies
  • Sub-Acute (SMRC)
    • Medically stable
    • Reasonably expected to benefit from rehabilitation
    • 16 years of age and above
  • Post-Acute Adult (FINR)
    • Achieved a stable level of medical recovery
    • 18 years of age and above
    • Functioning at a cognitive level on the Rancho Los Amigos Scale of IV or above
  • Post-Acute Pediatric (FINR)
    • Achieved a stable level of medical recovery
    • 4 to 17 years of age
    • Functioning at a cognitive level on the Rancho Los Amigos Scale of IV or above
  • Supported Independent Living (FING)
    • Achieved a stable level of medical recovery
    • 18 years of age and above
    • Functioning at a cognitive level on the Rancho Los Amigos Scale of IV or above
